Intake and Session Fees

45-minute Psychotherapy Session: $260

60-minute Psychotherapy Session: $335

75-minute Intake Session:   $400

Private Pay

I’m a private pay, out-of-network (OON) provider, which means that I’m not in-network with any insurance companies nor EAP programs, and payment is due after each session.

You will be asked to provide payment information before our intake.

You will be automatically billed the night after each session.

Benefits of Private Pay Psychotherapy

There are many benefits for clients who choose private pay therapy instead of using insurance, including:

  • Freedom to choose a therapist who is the best fit for your needs

  • Freedom for you and your therapist to determine the course and length of treatment

  • Increased confidentiality as your therapist will not have to share your treatment and diagnosis with an insurance company

  • No restrictions on online therapy

  • You can continue to work with the same therapist even if your insurance changes

Out-of-Network (OON) Insurance Reimbursement

I’m happy to provide monthly superbills that you could submit to your insurance company for partial reimbursement for OON psychotherapy if available based on your plan and deductible.

If you want to use OON benefits, I recommend you call your insurance provider and clarify what they cover. It is the client’s responsibility to understand their insurance plans and seek information about their company’s reimbursement process.

Please note that I am not contracted to accept Medicare Part B health insurance – it is not possible to submit your claims for reimbursement to Medicare.

Questions to Ask Your Insurance Company About OON Psychotherapy

You may find answers to these questions in your insurance Summary of Benefits or your insurance company’s website. I recommend calling your insurance company to verify your benefits. You can usually find this phone number on the back of your insurance card. Here are some questions to ask when you call:

  1. Do I have out-of-network (OON) outpatient mental health benefits?

  2. If I have out-of-network outpatient mental health benefits, what percentage or dollar amount will I be reimbursed for psychotherapy?

  3. Are licensed psychologists covered by out-of-network benefits?

  4. Is online therapy (aka telehealth, telemental health, or teletherapy) covered by out-of-network benefits?

  5. Do I have an annual deductible? If so, how much is it, and how much of my deductible has been met for this year?

  6. If I have a deductible, will I need to pay out of pocket for OON psychotherapy until that deductible has been met? After the deductible has been met, what percentage or dollar amount will I be reimbursed for OON psychotherapy?

  7. What documentation do I need to submit to receive out-of-network psychotherapy reimbursements?

  8. Do I need a referral from an in-network provider to see an out-of-network provider?

If it’s helpful, you can provide the following CPT codes to your insurance company:

  • 90791 for the intake

  • 90834 for 45-minute psychotherapy sessions

  • 90837 for 60-minute psychotherapy sessions

Insurance Coverage for OCD Treatment

Some insurance companies will provide enough out-of-network reimbursement to clients receiving treatment for OCD so that the client’s cost is the same or similar to if an in-network therapist was treating them due to a shortage of in-network therapists who provide specialized treatment for OCD through ERP (exposure and response prevention). I cannot guarantee that your insurance company will do this, and it is your responsibility to verify what they cover.

If you have been unable to find an OCD specialist that is in-network with your insurance, has expertise in ERP, and is currently taking new clients, then I recommend calling your insurance company to request that they provide you with reimbursement coverage to an out-of-network ERP for OCD specialist that makes your cost the same as what you would pay for an in-network provider. I recommend that you document your attempts to try to find an in-network therapist with expertise in ERP for OCD, document your communications with your insurance company, and be persistent with your advocacy for coverage.

Your insurance company may request that I sign a “single case agreement” to enter into a contract with them. I do not enter into "single case agreements" with insurance companies nor do I submit bills directly to insurance companies. However, I recommend that you tell your insurance company that because they have no one in-network available to treat OCD, you request that they reimburse you directly for your out-of-pocket costs.

Choosing a Session Length

We can discuss whether 45-minute or 60-minute psychotherapy sessions would be best for you at your consultation call and intake session. For individuals doing exposure therapy, such as exposure and response prevention (ERP) for OCD or exposure therapy for phobias and/or panic attacks, 60 minutes may be more beneficial to allow us to complete longer in-session exposure exercises.

If you’re seeking OON reimbursement for your therapy, I recommend asking your insurance company how much they reimburse for both 45-minute psychotherapy sessions (CPT code 90834) as well as 60-minute psychotherapy sessions (CPT code 90837) and if there are any restrictions on reimbursement for 90837 60-minute psychotherapy sessions.

Initial Free Consultations

As a first step to starting therapy with me, I offer a free 15-minute telephone consultation. The purpose of this call is to help us determine if I am a good fit for you.

I’ll ask you about your treatment needs, you can learn more about my approach to therapy, and I’ll answer your questions about me and my practice.

If we agree that I’m a good fit therapist for you, then I’ll send you pre-intake paperwork so you can schedule your intake!

Pre-Intake Paperwork

You must complete the pre-intake paperwork before your intake is scheduled and confirmed.

After our consult call, you will receive a link to the pre-intake paperwork. This paperwork includes consent forms for you to read and sign, payment information, questionnaires about yourself, and symptom questionnaires. You must complete this pre-intake paperwork before your intake appointment is scheduled. Once you complete your paperwork, you will receive an emailed link to our online intake session.

Please call me at 206-670-9662 or email me at info@drkellycaver.com if you have any questions while completing the pre-intake paperwork. I am not able to send text messages from my work phone.

Cancellations and No Shows

You may cancel your appointment without rescheduling a limited number of times (see below)  with no fee up to 72 hours before your appointment. You can cancel through the online portal, send an email, or leave a voicemail (which is time-stamped).

To support consistency and continued progress, weekly clients are limited to no more than two cancelations without rescheduling within six months, and biweekly and monthly clients are limited to no more than one cancelation within six months. You will be expected to pay the full fee if you cancel more often than this without rescheduling, regardless of the reason for cancelation. To not be counted as a cancelation, rescheduling must occur the same week for weekly clients, the same two-week period for biweekly clients, and the same month for monthly clients.

You will be expected to pay the full fee if you cancel within 72 hours or do not show up for your appointment. Exceptions may be made for emergencies (i.e., death or birth in the family, an accident) or serious illnesses or injuries of yourself or someone you care for. Work or travel-related conflicts will not be exceptions to this policy. These policies are necessary because a time commitment is made to you and is held exclusively for you.
